Health status and academic achievement have been found to be linked. When students have poor health status, they are at increased risk for poor academic outcomes. The goal of the school-based health center is to mitigate the effects of poor health on a child’s academic performance, by improving access to health care, as well as healthy behaviors and outcomes, for students within their school. The District will contract with the Health Federation to continue to provide a care management coordinator to operate the school based health center at Gideon, serving students at Gideon, Wright, WD Kelley, and Blaine schools. The contractor has extensive knowledge and background in health care human resource management. The care management coordinator will have responsibility/ accountability for leading and managing the daily operations of the school-based health center, and delivering primary care, managing chronic conditions, and treating short-term illness for students attending the four designated schools.
